do I need to write affidavit of loss on my own, or does the notary do it for me?

i have lost most of my receipts and bank is requesting for an affidavit of loss.

I will send you the draft of the affidavit thru email, Messenger, Viber, Whatsapp, etc. then you can download it and print as many copies as you want.
